A. Procedural Forms Requesting Assistance

The Family Form A is available to request assistance from the Self-Help Office. The Form C is only for use in Child Support cases involving Child Support Enforcement (the Department of Revenue). Read the instructions carefully on each form! You must file a separate form with the Clerk of Court each time you request assistance.


B. Common Procedural Forms Required in Family Division Cases

You may receive a notice from the Self-Help office stating that you need one or more of these forms filed with the Clerk of Court before your case can proceed to a final hearing. Other required forms can be obtained elsewhere on this website, or on the Florida Court website at www.flcourts.gov.


C. Various Motions to Request Action by the Court

Each form has instructions indicating the proper use for each form. Read and follow the instructions carefully!

* Note: Respondents who wish to file any motion in a case that involves Child Support Enforcement (Department of Revenue) should use the Respondent’s Packet for Cases Involving Child Support Enforcement. Common motions specific to child support matters are also available in the Respondent’s Book for Cases Involving Child Support Enforcement. Please do not use the following forms.


D. Petitions and Supporting Documents

The Petitions and supporting documents for the most common Family Division proceedings are available on the Family Division Form Packets Page the and at the Clerk of Court in packets.  Individual forms can be obtained on the Florida Courts website at www.flcourts.gov.


E. Answers and Supporting Documents

You must file a response called an “Answer” within 20 days after being served with legal papers and a summons. The following are some of the most common Answers, but they may not be applicable to your case.  You can review and obtain the Answers and supporting documents for the most common Family Division proceedings on the Florida Courts website at www.flcourts.gov. Please contact your local clerks office as there may be an additional fee associated with the filing of the documents below.
